GLP-1 Prescription Eligibility: BMI Requirements and Who Qualifies in 2026

BMI 30+ or 27+ with comorbidities. But the rules vary by medication and provider. Complete eligibility breakdown for 2026.

Published June 2026 · Evidence-based · Not medical advice

GLP-1 prescription eligibility has expanded significantly in 2026 with new medications and coverage pathways. Here's who qualifies, for what, and through which channels.

The Core Requirements

For obesity (weight loss indication): BMI ≥30, or BMI ≥27 with at least one weight-related comorbidity (type 2 diabetes, hypertension, dyslipidemia, obstructive sleep apnea, cardiovascular disease).

For type 2 diabetes: Diagnosis of type 2 diabetes. No BMI requirement — GLP-1s are prescribed for glycemic control regardless of weight.

Disqualifiers

You will not qualify if you have a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2), are pregnant or breastfeeding, have a history of pancreatitis, have type 1 diabetes, or have an active eating disorder (anorexia, bulimia).

What If Your BMI Is Borderline?

If your BMI is between 27 and 30, you need a documented weight-related comorbidity. Many people in this range have at least one — prediabetes, elevated blood pressure, joint pain from excess weight, or sleep apnea. Your prescriber will evaluate your full clinical picture, not just the number on the scale.
