Medicare GLP-1 Bridge in detail

The Bridge is a Part D program — it covers Wegovy and Zepbound at obesity doses through your existing Part D plan. Cost-sharing: standard Part D cost-sharing applies until you hit the $2,000 annual out-of-pocket cap (per the Inflation Reduction Act), after which your out-of-pocket drops to zero. For low-income subsidy (LIS) beneficiaries, cost-sharing is minimal from the first fill.

The sunset risk

The Bridge expires December 31, 2027. Patients who start treatment and respond well face coverage loss at the sunset date unless Congress acts or CMS extends the demonstration. The long-term discontinuation data (SURMOUNT-4, STEP-1 extension) shows significant weight regain after stopping — making a two-year coverage window with an uncertain renewal a real clinical planning issue.

Private insurance coverage (Medigap + Medicare Advantage)

Medicare Advantage (Part C) plans may offer more comprehensive GLP-1 coverage than Original Medicare — some plans include obesity drug coverage beyond the Bridge. If you have Medicare Advantage, check your plan's formulary specifically for Wegovy and Zepbound at obesity doses. Medigap supplemental policies don't typically add drug coverage beyond Part D.
