GLP-1 Prescription Requirements: BMI, Conditions, and What Actually Qualifies You

The FDA-Label Criteria
For weight management specifically, GLP-1 medications are FDA-approved for adults with BMI ≥ 30 (obesity) regardless of other conditions, or BMI ≥ 27 (overweight) with at least one weight-related comorbidity: type 2 diabetes, hypertension, dyslipidemia (high cholesterol/triglycerides), obstructive sleep apnea, or cardiovascular disease.
How to Calculate Your BMI
BMI = weight (kg) ÷ height (m²). For practical reference: a 5'10" man weighing 210 lbs has a BMI of 30.1 — just over the threshold. A 5'4" woman weighing 160 lbs has a BMI of 27.5 — qualifying with a comorbidity.
BMI is an imperfect measure (it doesn't distinguish muscle from fat), but it's the standard telehealth platforms use for qualification. If you're close to the threshold, most providers will evaluate your full clinical picture rather than rejecting based on a fraction of a BMI point.
Common Disqualifiers
Personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN2) — this is an absolute contraindication for all GLP-1 medications. History of pancreatitis may require additional clinical review. Pregnancy or active planning for pregnancy within 2 months. Current use of other GLP-1 medications or insulin (requires coordination, not necessarily disqualifying). Severe gastroparesis or GI conditions that could worsen with delayed gastric emptying.
What If You're Below BMI 27?
GLP-1 medications are not FDA-approved for cosmetic weight loss in people with normal BMI. Some providers may prescribe off-label for patients with BMI 25–27 and metabolic risk factors, but this is provider-dependent and not standard practice. If your BMI is below 25, GLP-1 medication is not clinically appropriate.
